Please note that the application window for GA Childcare Reimbursements will close on Friday, January 17 at 11:59 PM. Instructions and a link to the form can be found HERE. Late applications cannot be accepted.

• This application covers the period of August 23, 2024 to January 7, 2025.
• We require itemized receipts with service dates that show the cost of child care provided during the period covered by the application.
• Please note you will need to use a Gmail account to upload the required documentation as the application is hosted as a Google Form.
• If you need to make any changes after submission please email us with your request. Duplicate applications will not be accepted.
